What to include
- The dates. The first and last day you expect to be away. If you do not know the end date yet, give your best estimate and say you will update them.
- A short reason. "I am unwell and not fit to work" is enough. Add more only if you want to.
- Evidence, if needed. Say whether a fit note or medical certificate is attached or will follow.
- Cover for your work (employees). Who is handling anything urgent, and where your notes are.
- A polite close. Thank them and say you will keep them updated.
For employees
Most workplaces want a phone call or message on the first morning, before your shift starts. The written
application then gives a record of the dates. Keep it short and factual. If you are off for longer, many employers
ask for a doctor’s note after a set number of days; check your contract or staff handbook.
If you manage people or deadlines, say who is covering. It reassures your manager and means fewer calls while you
are trying to rest.
For students and parents
Schools in India, Nigeria, Pakistan and many other countries expect a formal application addressed to the principal
or class teacher, with a subject line and "Respected Sir/Madam". The student tool uses that format. Include your
class and roll number so the office can find your record quickly.
Parents writing for a younger child should say who the child is, their class, the dates and that they will catch up
on missed work. University students writing to a tutor can use the student option and add their course and
matriculation or registration number.
Your privacy
Health information is private. The tool only puts in your letter what you type in the "Reason in your own words"
box, and your draft is saved in your own browser (and in your account only if you sign in and choose to save it there). Use "Start over" to clear it on a shared computer.
Sick leave and sick pay rules vary by country, employer and school. This is general information, not legal or
medical advice. Check your contract, handbook or school policy, and see a doctor if you are worried about your
health.
